CLEARANCE/FORMS
Information for Participation in Activities
INSTRUCTIONS:
Go to athleticclearance.com to begin creating your account. You will need to create an account in order to begin. Once you have done that, please click on the button that says “Start Clearance Here”! You will then select the year, school, and activity for your student. Please be sure you select the correct school year. You will need to do a separate clearance for each student in your house. Only one parent account is needed regardless of how many students you have at TOHS.
All forms require signatures which can be done as electronic signature of both student and parent. The doctor’s physical form does need to be printed from the site, filled out in its entirety, signed and dated by the doctor, and then uploaded back into the site.

Please check to ensure the doctor has written the date of the physical on the form. We also require a copy of the students’ medical insurance card to be uploaded for clearance. Upload physical and it will be automatically attached to your account.
Once all forms have been completed online, you will receive a confirmation that your student is clear for their season. This clearance is generated by the activities department once we have reviewed all forms as being complete.

The physical exam is valid for 12 months from date of exam.
Students are not eligible to practice, or participate in any sport or activity until all digital forms are completed and the physical and proof of medical insurance is uploaded to AthleticClearance.com.
***Please be advised that Activities and Front Office staff will not fax or email forms to/from parents or doctors.
